Terms and Conditions of Participation in Dental for Children PSA Training Courses
§1 General Provisions
1. The organizer of the training courses is Dental for Children PSA with its registered office in Reda, ul. Szkolna 6/24, hereinafter referred to as the "Organizer."
2. The training courses take the form of theoretical lectures and workshop activities.
3. Information about the training courses, dates, and participation conditions are published on the website: https://dentalforchildren.pl/pages/szkolenia
§2 Applications and Registration Rules
1. Participation in the training course is registered electronically via the form available on the Organizer's website and the Development Services Database (Baza Usług Rozwojowych).
2. Submitting the application form is equivalent to acknowledging and accepting these regulations.
3. The deadline for submitting applications is 14 days before the planned start date of the training course.
4. The number of places in the training course is limited. Admission to the list of participants is determined by the order of applications and timely payment of the advance.
§3 Fees and Payment Conditions
1. The condition for reserving a place in the training course is the payment of an advance of PLN 1000 (one thousand zlotys), regardless of the total cost of the training course.
2. The advance should be paid within 3 business days from the moment the application form is sent.
3. The remaining part of the training fee must be settled no later than 14 days before the start of the training course.
4. Failure to pay the full amount within the required period means resignation from participation in the training course.
5. In the transfer title (for both the advance and the remaining payment), please enter: "Training name, training date, participant's name and surname."
§4 Invoicing
1. The Organizer documents payments as follows:
a) After the advance payment (PLN 1000) is credited, an advance invoice is issued,
b) After the remaining amount is settled, a final invoice is issued.
2. Invoices are sent electronically to the e-mail address indicated in the application form or via the KseF system.
3. Registration for the training course means consent to receive VAT invoices in electronic form.
§5 Resignation from participation in the training course
1. Resignation from participation in the training course must be reported by e-mail to: szkolenia@dentalforchildren.pl
2. In case of resignation:
a) More than 30 days before the training course – 100% of the paid amount is refunded,
b) From 30 to 21 days before the training course – 50% of the paid amount is refunded,
c) In the event of a Participant's resignation from a training course funded or co-funded under the Development Services Database (BUR), the Organizer is entitled to deduct a handling fee of PLN 200.00 from the amount to be refunded.
d) Less than 21 days before the training course – the payment is non-refundable.
3. Failure to participate in the training course without prior resignation does not entitle to a refund of costs.
§6 Complaints
1. The Participant has the right to file a complaint regarding the training service if its execution (in particular, time, place, or materials) was inconsistent with the offer.
2. Complaints should be submitted in written or electronic form.
3. Complaints are considered individually within 14 business days from the date of their receipt by the Organizer.
§7 Personal Data Protection (GDPR)
1. The administrator of participants' personal data is Dental for Children PSA with its registered office in Reda, ul. Szkolna 6/24.
2. Participants' personal data is processed for the purpose of:
a) Realizing the registration process and organizing the training course,
b) Issuing accounting documents,
c) Contacting the participant on organizational matters.
3. Providing personal data is voluntary, but necessary for participation in the training course.
4. The participant has the right to:
a) Access their data
b) Rectify it
c) Erase or restrict processing
d) Object to data processing
5. Personal data will not be transferred to third parties, except for entities handling payments and accounting, to the extent necessary for the provision of services.
6. Data will be stored for the period necessary to achieve the purpose and in accordance with applicable legal provisions.
§8 Participants' Image
1. During the training course, photographic materials and video recordings may be taken.
2. These materials may be used by the Organizer for marketing purposes, in particular on the website and in social media.
3. By participating in a group training course, the participant agrees to the free use of their image.
§9 Liability
The Organizer is not responsible for participants' personal belongings left unsupervised during the training course.
1. The participant takes part in the training course at their own risk.
2. The Organizer is not responsible for how the knowledge gained during the training course is used by the participant.
3. The participant is obliged to comply with safety rules and the instructor's instructions during workshop activities.
4. The Organizer is not responsible for the consequences of using knowledge acquired during the training course in clinical practice.
§10 Conditions of Participation
1. The training courses are intended for dentists.
2. The participant declares that they have appropriate qualifications to participate in the training course and perform the activities covered by the training program.
3. In the case of training courses carried out via the Development Services Database (BUR), the participant undertakes to complete the survey in the BUR system within 3 calendar days from the end of the training course.
§11 Organizational Changes
1. The Organizer reserves the right to introduce changes to the training program, schedule, and instructor due to organizational reasons.
2. These changes do not constitute grounds for resignation from the training course or for claims against the Organizer, unless they significantly affect the substantive scope of the training course.
§12 Final Provisions
1. These regulations are effective from the date of their publication.
2. For matters not regulated by these regulations, the provisions of the Civil Code shall apply.
